Consortium Service Request Form

The Healthy Communities Consortium, funded by the Ministry of Health Promotion as part of the Healthy Communities Fund, helps community organizations and groups that are building healthy communities through a wide range of activities in areas such as healthy eating, physical activity, sport and recreation, injury prevention, mental health promotion, tobacco use and substance and alcohol misuse.

The Healthy Communities Consortium (HCC) is a collaborative effort of four organizations that collectively have more than 25 years experience in health promotion.
