4. RESOURCES
a) Informative Ministry of Health Promotion Websites
The Ministry of Health Promotion (MHP) has several web sites designed to inform the public about the goals and mandate of the Ministry. They provide useful information, tools and resources to promote healthy living. Collectively they support all five pillars of the Ottawa Charter: creating supportive environments, re-orienting the health system, strengthening community action, building healthy public policy, and developing personal skills. Visit these sites at:
b) Primer to Action - Social Determinants of Health
Overwhelmingly positive feedback was received after the launch of the "Primer to Action - Social Determinants of Health" in April 2007, by Health Nexus and the Ontario Chronic Disease Prevention Alliance (ODCPA). However, the partners recognized that there was further work to be done. Many public and community health intermediaries in Ontario understand the relationship between chronic disease and social determinants of health, while others understand intuitively that social conditions influence health. However, very few Ontario organizations working in health, social service, educational and community change effectively address SDOH. To address this gap between knowledge and action, the Ontario Chronic Disease Prevention Alliance and Health Nexus collaborated on the second phase of this project - Priming Action - which has been funded by the Public Health Agency of Canada, Ontario - Nunavut. The revised and improved Primer to Action - Social Determinants of Health is now available.
