Helping to Build Healthy Communities
The Healthy Communities Consortium, funded by the Ministry of Health Promotion as part of the Healthy Communities Fund, supports community groups, organizations and partnerships who are working toward building Healthy Communities throughout Ontario.
Who We Are
The Healthy Communities Consortium is a collaboration of organizations that, together, have more than 25 years of experience working with community groups, coalitions and organizations on community-based health promotion approaches.

- Heart Health Resource Centre (HHRC)
- OHCC Healthy Communities Support Services (OHCC)
- Ontario Drug Awareness Partnership (ODAP)
How We Work
Consortium members work collaboratively with a range of organizations and resource centre, including former members of the Ontario Health Promotion Resource System (OHPRS), to build on existing resources and expertise and draw upon evidence informed models, tools and strategies.
What We Offer
The Consortium provides a variety of services and supports delivered through on-site, email or phone consultations, workshops, webinars and resources on such topics as:
- Program development, planning, implementation and monitoring
- Strategic planning
- Evaluation
- Partnership building, coalition development and network mapping/analysis
- Community development, mobilization and engagement
- Policy development and change
- Healthy Communities process and principles
- Content expertise in the priority areas and populations within the Healthy Communities Ontario approach*
- Referrals to additional resource centres and organizations
Who Can Access Our Services
- Those who are applying for or are interested in applying for a Healthy Communities Fund grant*;
- Those who have received a Healthy Communities Fund grant*;
- Healthy Communities Partnerships; and
- Those who are working with or are interested in working with a Healthy Communities Partnership.
