Online Survey for Healthy Community Initiatives

As part of our “Healthy Communities: An Approach to Action on the Determinants of Health in Canada” project we have developed an online survey to find out about more Healthy Communities initiatives and action on the determinants of health. This 15-20 minute survey is designed to help us learn about Canadian Healthy Communities initiatives in Québec, Ontario, New Brunswick and British Columbia.

About the Project

The Healthy Community movement is a dynamic force in many communities across Canada that brings together groups from many sectors to plan and implement strategies to enhance community well-being and address complex community issues. This project aims to demonstrate that the Healthy Community approach is an effective way to address chronic disease prevention.
